Submission of Medical Report / Insurance Claim
In order to comply with the PERSONAL DATA PROTECTION (PDP) Act 2010, all submissions for medical reports/insurance claims / SOCSO / legal reports / KWSP, the following procedures must be followed. This is to protect both parties between Hospital and the patient.
A. If the applicant is a patient
- Insurance form/request letter
- Consent to release information
- Copy of patient’s I/C
B. If the patient is below 18 Years Old
- Insurance form/request letter
- Consent to release of Patient Information
- Copy of patient’s I/C or birth certificate
- Copy of parent’s I/C (either one)
- Processing fee (please check with HIMS staff)
C. If the patient is deceased
- Insurance form/request letter
- Consent to release of Patient Information
- Copy of relative’s I/C (Registered relative only. Eg: Spouse, Child)
- Original death certificate of patient
- Processing fee (please check with HIMS staff)
D. If from an insurance company/relative/legal firm
- Insurance form/request letter
- Consent release of Patient Information.
- A copy of the patient’s and requestor’s I/C
- Processing fee (please check with HIMS staff)
E. Consent Form
- Consent to release information
This consent must be signed by the patient except if the patient is a minor (under 18 years of age) or has a mental incapacity to consent for the release of information, or is deceased.
